IRB approval process (IPSR)
Following is the Institutional Review Board (IRB) process (with Institute for Population and Social Research: IPSR)
**Please noted that
1.) ALL students who will submit the IRB approval, must attend the IRB training program or pass the on line CITI program
(Because evidence of passing the training will be required in the submitted documents for reviewing.)
2.) ALL research projects must be submitted for IRB approval prior to the data collection stage.

2. Submission

(Please submit 2 weeks prior to the scheduled meeting)
3. When your request is approved
IPSR will send the Certificate of Approval (COA) to you via E-mail when the committee approve your request.

- Informed consent document (Download: Th / Eg )
- Research tools (i.e. questionnaire, interview / observation guideline)
- Letter of permission to use the data (if secondary data is used)
- Transaction record of payment for review
- Certificate Training in Human Research EthicsOthers (specify): CV of Advisor and CV of student
- Administrative fee for each type of research is summarized as follows:
